Under-Eye Wrinkles and Drooping Eyelids Are Not the Same Problem
Why under-eye lines in your 20s and drooping eyelids in your 50s do not get the same treatment

Someone who searched "how to get rid of under-eye wrinkles" and someone who searched "drooping eyelid treatment" will hear completely different things in the consultation room. Both are the eye area by name, but the layer at fault is different.
We look at the eye area in three layers
At ReYeonK, the eye area is divided into layers.
Surface — fine lines that appear when you smile. Wrinkles on the skin surface.
Dermis — texture that has thinned and slackened as firmness declines. Fine lines start to remain even when you are not smiling.
Deep layer — eyelids that have descended and outer corners that droop. Not the skin, but what lies beneath it, has come down.
No single treatment covers all three layers. So the EyeThera programme is divided into Eye Botox (surface) · Virtue Eye (dermis) · Eye Ultherapy (deep layer), and only the layers that are needed are included.
Under-eye wrinkles — your 20s and your 40s are different
In your 20s, under-eye wrinkles are mostly surface lines that appear only when you smile. There is no reason to touch the deep layer. Softening the expression muscles (Eye Botox) comes first, and the change shows within 2 to 7 days.
That said, the eye area is narrow and delicate, so even botox is placed with more precise injection points and doses. A product that tends to spread must not be used widely around the eyes — that is why.
After 40, under-eye wrinkles remain even without smiling. Dermal firmness has declined, so treating the surface alone is not enough, and something that builds collagen in the dermis — like Virtue Eye — comes in. Improvement is gradual over 4 to 12 weeks.
For those whose concern is discolouration, fine lines and dark circles directly under the eye, we sometimes recommend the Rituo Eye + Eye Botox combination; for those who come with hollowing lower down under the eye, we sometimes use under-eye filler. Whether it is wrinkles, colour or hollowing decides what is used.
Drooping eyelids — what treatment can and cannot do
Eyelids that have descended and make the eyes look smaller are a deep-layer problem. This is where Eye Ultherapy comes in — depth and intensity reset to eye-area standards, the eye protected throughout. Results appear over 4 to 12 weeks and last 6 to 12 months.
These days some patients specifically ask for Rejuran Eye on the eyelids (despite the discomfort). And for those with a deeply hollowed upper eyelid, we sometimes recommend upper-eyelid filler, because of how tired it makes the face look. Sagging and hollowing are different problems, so the same eyelid can go in different directions.
If the problem has gone beyond what treatment can resolve, however, we recommend surgery. Where that line lies is something we tell you in consultation, after looking at your eyes.
So the first thing we do
Because the eye area is hard to reverse, we decide what can be left out before deciding what to do. We take photographs and look at the layers together in consultation. Adding a layer that is not needed only adds cost and recovery.
